Service Eligibility

To qualify for our services, patients must be under the care of physician, homebound or otherwise, receiving services under a plan of care established and periodically reviewed by a physician, be in need of skilled nursing care, physical, speech, and occupational therapy on an intermittent basis.

Skilled Nursing Services

  • Making the initial comprehensive assessment visit to the patient.
  • Provide the services that require more specialized nursing care which include and not limited to:
    • IV Therapy
    • Tube Feeding/Tracheostomy care, Colostomy Care
    • Wound Care and Treatment
    • Respiratory Equipment Management and Teaching
  • Reevaluating their needs regularly.
  • Initiation and revising the nursing plan of care.
  • Planning for preventative and rehabilitative care.
  • Preparing clinical notes and informing physicians and Others participating     in the patient’s care of changes as they occur.
  • Disease Process Management
  • Medication Management including administration of IV, IM and SC Medications.

Skilled Nursing Services

  • Assess patients/families ability to cope up with social, psychological, cultural, and medical issues resulting from an illness.
  • Provide counseling.
  • Provide community resources as needed.

Physical Therapy Services

  • Provide evaluation and treatment of patients rehabilitation need.
  • Therapeutic exercises for muscle conditioning and strengthening to regain functional mobility and gross motor skill.
  • Home safety assessment and instruction.

Occupational Therapy Services

  •  Provide training for activity of daily living to maximize the patient’s functional independence within his or her home.
  • Evaluation and Training in adaptive equipment.
  • Studies, evaluates, and records patient’s activities and progress.

Speech Therapy Services

  • Maximizing a patient functional independence in communicating and swallowing.
  • Increasing the patient’s and caregiver’s knowledge of speech, language and swallowing disorders, and appropriate treatment processes.

Home Health Aide Services

Assist patients with personal care needs.

  • Plan and prepare meals and assist in feeding patients as directed.
  • Light housekeeping.
  • Helps patient maintain food personal hygiene and assist in maintaining a healthful safe environment.

Pharmaceuticals & Laboratory Services

Coordinate delivery of Prescription Drugs, or Laboratory services ordered by   doctor ‘s done n patient’s home.
